A program for all-inclusive care for the elderly (PACE) that supports individuals 55 years and older that are deemed nursing home level of care. LIFE is a nursing home alternative that allows patients to remain living in their homes while providing medical oversight. There is no cost to the program if financially eligible, including medications. Additional services include occupational therapy, physical therapy, transportation to medical appointments and recreational therapy at the day health center.
