Description:

Description:

Standardized and coordinated assessment and referral service for everyone who is seeking services from the network of homeless services in Mifflin, Juniata and surrounding counties. Clients will complete an assessment with an intake caseworker and will then be referred to the appropriate program.

Description:

Apartment units where lease is held by service provider and sublet to consumer; Consumer pays portion of rent based on income; Support services provided by community supports.

Community Residential Rehabilitation Services

Description:

Residential program for individuals with serious mental illness; 24/7 staff; temporary assistance to improve activities of daily living; individuals have autonomy to come and go.

Mobile Medication Management

Description:

Time limited intensive program aimed at assisting individuals in learning to manage their medications. The goal is for the individual to be able to independently take medications, understand side effects, and know the purpose of the medications they are taking.
